Reporting Varicella (Chickenpox) Outbreaks

Per Iowa Aministrative Code 641 Chapter 1, healthcare facilities, providers, and laboratories shall immediately report any confirmed or suspected case of a common source epidemic or disease outbreak of unusual numbers to the Center for Acute Disease Epidemiology at (800)362-2736.

Recommendations for Pregnant Women:

Varicella vaccine is contraindicated for pregnant women. Pregnant women without evidence of immunity to varicella who have been exposed to varicella should receive varicella-zoster immune globulin. These individuals should contact their healthcare provider for additional information.

Premature babies exposed to varicella, specifically:

Hospitalized premature infants born at ≥28 weeks of gestation whose mothers do not have evidence of immunity, or
Hospitalized premature infants born at < 28 weeks of gestation or who weigh ≤1,000 grams at birth regardless of their mothers’ varicella immunity status

Should also receive varicella-zoster immune globulin. These individuals should contact their healthcare provider for additional information.

Recommendations for immunocompromised persons:

Persons with a weakened immune system that are not immune to varicella should receive varicella-zoster immune globulin. These individuals should contact their healthcare provider for additional information.

Recommendations for Unvaccinated or Undervaccinated Persons:

Unvaccinated or undervaccinated individuals should be brought up to date on their varicella vaccination as soon as possible as vaccine can prevent or significantly modify outcome of disease.

Individuals that cannot receive vaccine should be evaluated to determine if they are at high risk of disease. Those not at high risk should be considered for antiviral drugs to prevent or modify illness.

In outbreak situations, unvaccinated children who are ineligible for vaccination may be excluded from school or childcare up tot 21 days after the last case is identified.

To control spread:

Infected individuals should remain home until all lesions have crusted.

During outbreaks, exposed susceptible individuals who refuse vaccination or are ineligible for vaccination should remain home from the 8th until the 21st day post-exposure.
